This form should be used anytime you are of the belief that you will not complete your assignment in the time allotted.  This form is to our benefit, not managements.  That is why management tries to discourge you from using it.  Always request a copy.  If management does not give you the form, request to see your steward.  If management does not give you your copy back as requested, request to see your steward.  If your steward is not available or you do not have one, call the branch office (216-273-7030).
